Group Benefits Plan

CPMA BFF IndiPlan

For those without staff or for a group of 1 – 3 Individuals, Couples and Families. There is no Medical questionnaire No Deductibles or Dispensing fees.

Plan highlights:
• 80% reimbursement on Prescription Drugs with an annual max of $2500 per person.
• 80% reimbursement for paramedical Practitioners $300 / person/practitioner/year
• 100% Reimbursement for Private Hospital coverage
• 100% coverage for Eye Exams
• 80% Coverage for Basic and Restorative Dental, Periodontics and Endodontics to $1000
• 100% Out of country coverage for trips up to 90 days.

Plan cost:
Single – $112/month
Couple – $224/month
Family – $267/month

CPMA BFF IndiElite Plan

For those without staff or for a group of 1 – 3 Individuals, Couples and Families. There is no Medical questionnaire No Deductibles or Dispensing fees.

• 80% reimbursement on Prescription Drugs with an annual max of $5000 per person.
• 90% reimbursement for paramedical Practitioners $400 / person/practitioner/year
• 100% Reimbursement for Semi-Private Hospital coverage
• 100% coverage for Eye Exams
• 80% Coverage for Basic and Restorative Dental, Periodontics and Endodontics to $1500
• 100% Out of country coverage for trips up to 90 days.

Single – $135/month
Couple – $266/month
Family – $327/month

CPMA BFF Enhanced

This plan is for groups of 3+ practitioners wanting a true employee benefit plan for themselves and their staff.

Plan highlights:
• $25,000 Critical Illness Coverage
• 80% reimbursement on Prescription Drugs with an annual max of $5000 per person.
• 80% reimbursement for paramedical Practitioners $300 / person/practitioner/year
• 100% Reimbursement for Semi-Private Hospital coverage and other Major Medical
• 80% coverage for Eye Exams Glasses and Contacts to $300 per person every 24 months
• 80% Coverage for Basic and Restorative Dental, Periodontics and Endodontics and
• 50% Dentures and Major restorative a combined maximum of $1000
• 100% Out of country coverage for trips up to 60 days.

Single – $105/month**
Family – $267/month**
